This is Doom 64 Retribution Level 18: Spawned Fear - done with all kills and secrets on Watch Me Die-difficulty.

Spawned Fear is the eighteenth level of Doom 64. This map keeps you backtracking a lot, because many of the Keys and doors are on the other side of the map, which is pretty annoying, but fortunately Spawned Fear is a generous map with Armor and Health, but you still have to be on your guard because there are numerous traps around the level. If you're not prepared for them, they can end your days pretty quickly.

I don't know which Doom 64 level has the highest number of Hell Knights, but if I'd have to take a guess, I'd say its this level. After all the backtracking, you'll be rewarded big time if you pay attention, because there is a secret Exit here, leading to Level 31, allowing you to obtain the last Demon Artifact.
