As for the Monster Hunter bit: There is no direct evidence for whoever pitched the original idea of Monster Hunter. But I did discover that (as per The Guardian) Monster Hunter was part of an initiative from Capcom's Production Studio 1 to develop three network focused games on the PlayStation 2. And considering Ryozo was head of the entire online planning department at the time (credited as such under the 3 games: MH, Auto Modelista and RE Outbreak), he either had a hand in the pitch directly or in getting it approved instead of Itsuno's idea.
I will reiterate that I DO NOT THINK MONSTER HUNTER RIPPED OFF DRAGON'S DOGMA. It is just an incredibly tragic (and hilarious) moment in history.
