In this mod review I cover a mod that adds a new type of enemy to Fallout 4. This is the Defective Synths mod which adds new ghoul like synths to the commonwealth. The lore is that synths over exposed to radiation go nuts and function very similarly to ghouls. Thats where you will find this enemy, generally hanging out in packs of ghouls. These enemies are visually distinct and actually horrifying to look at, as they are a gross combination of metal robotic parts and bones and muscle tissue. There are a couple different variants you can run into it. From the basic feral synth to crawlers who pull themselves to you with no legs. There are also harder variants like the Psycho Raider Synth who functions like a feral assaultron like those enemies weren't terrifying already. These enemies are quite tough to so not only do they add visual variety they actually increase the challenge of the game, and it's something you would need to pay attention to every time you ran into ghouls.
