In this mod review I cover a mod that changes feral ghouls into something much worse. Feral Ghouls will change into one of five new variants of these hell things that can change the vibe of the game and make you face brand new horrors. As far as I know it doesn't change combat behavior but just the sight of these things really does get into your head.
