Whenever people say that a firearm "went off" what that really means is a negligent discharge. Firearms will not just fire unless someone has a finger on the trigger. It could happen if the trigger catches on some object or if the rifle, shotgun or pistol is dropped but that is very unlikely. Especially since most modern firearms have various types of internal safety mechanisms.

With a responsible mindset, awareness and adherence to safety rules dangerous accidents can easily be avoided. The best safety is trigger discipline and conscious control of the muzzle direction.
