In order to earn the No Thoughts Empty Head achievement, you must clear a stage while only ever choosing the top option when leveling up. This can be a bit tricky, since you are basically at the mercy of RNG of what your abilities and stat boosts will be in your run. I would suggest waiting until you level up some of the permanent upgrades in the shop first.

I would suggest doing this on the first level, the Grassy Plains, as the enemies are the easiest there, and since RNG will determine how your run goes, you may as well make it easier on yourself.

Play through the stage, and just keep picking the top option. Remember, rerolls aren't allowed either! I haven't unlocked rerolls yet, so that's one thing I didn't have to worry about, but just make sure to not use any while attempting this. Once you have beaten the boss, the achievement will unlock!
