There is a simple pattern to playing spy, if you can recognize it. Spies seem random until you work out what they are doing.

Also for those spies running into enemy teams with a disguise using a dead ringer. Be sure to throw on a friendly disguise not and enemy one, that way you wont be picked up as obviously. You will see a spy running in with a medic disguise, he is expecting to be triggered but the backstab gets through that.
