Prepare for the fight with Necrophage Oil and Quen, then work to position yourself outside of the crypt so you have room to dodge. The Grave Hag is very fast so use Yrden to slow her down so you can more easily fight her with your sword. Use the Black Blood Potion so that if the Grave Hag wounds you damage will be reflected back at her. If you are using the below build you can continue to use potions to heal you so you can use your Stamina for Yrden instead of Quen.
