Use Aard to interrupt his attacks and spells then hit him with a strong attack followed by one fast attack. Occasionally with good positioning, such as when Letho is against a wall, you can get additional fast attacks in. Once Letho can block you, dodge his attacks before using Aard again to repeat the attack chain. When Letho uses Quen, just avoid him as it will expire in a few seconds. You just have to get Letho to half health before a cutscene triggers and the battle is over.
