A common controller gamepad d-pad glitch on older games like Fallout 4 is the d-pad skipping entries when you try to select them. It's like you're pressing the d-pad twice, up and down or side to side, and it makes it impossible to choose some menu options, or the right weapon in the quick-access menu.
The fix is to change the order of controllers in the settings menu and restart the game.
