To transfer game saves from the PS4 version of Fallout 4 to the PS5 version, you have a few options:

Data Transfer via the Internet:
Make sure you’re signed in to the same PlayStation account on both consoles.

Ensure both consoles are connected to the same Wi-Fi network.
Go to Settings on your PS5, then navigate to System - System Software -Data Transfer.

Follow the prompts to transfer saved data and game files.
Transfer Saves via PlayStation Plus:

Go to Settings -Application Saved Data Management-Saved Data in System Storage on your PS4.

Choose Upload to Online Storage to save your game data to the cloud.
On your PS5, go to Settings - Application Saved Data Management - Saved Data in System Storage.

Select Copy to USB Storage Device and choose the games and save files you want to bring over.

Insert the USB device into your PS5.
Upgrade PS4 Games to PS5 Versions and Transfer Saves:
Update the PS4 game to the latest version.

Open the save menu in Fallout 4 on your PS4.
Upload your save to the cloud from the save menu.
Close the game and open the PS5 version of Fallout 4.
In the save menu, there will be an option to download your cloud save
