Bruce Banner, who has been on the run for several years and who in his rage transforms into the super-strong green-skinned monster #Hulk, is contacted by his wheelchair-bound former mentor, Jeffrey Crawford, who, in his own words, has invented a device that can free Bruce from his curse - gamma - a sphere that should absorb gamma radiation from Banner’s body. Crawford also warns Bruce that some time ago his laboratory was raided by Atheon, led by General John Riker, who wants to create an army of hackle-like monsters. The Vandal takes the sphere and escapes the laboratory; The #Hulk gives chase, pursued by Riker's soldiers. After a long chase through rooftops and sewers, the Hulk encounters the energy vampire Half-Alive, who can absorb the life force of living beings upon touch. Having dealt with Half-Dead, the #Hulk finds out from him that Crawford went to Alcatraz, where they are holding a hostage who will be immediately killed if the Hulk appears there.
Secretly, Bruce sneaks into the laboratory and launches a device connected to a gamma sphere, which actually absorbs gamma radiation from Bruce's body.
As the process comes to an end, Crawford suddenly opens the sphere and absorbs the radiation and transforms into the monster Vandal. Stricken by the betrayal, Bruce himself turns into the Hulk.
Hiding from security, Bruce manages to sneak into Alcatraz's main facility and disable the gamma radiation detection system. From the CCTV cameras, Banner sees that the criminal who has taken over the island, nicknamed the Leader, is creating Hulk-like soldiers using a camera powered by a gamma sphere. Bruce goes through the cameras to the laboratory where the hostage scientist, his beloved Betty Ross, is working. Noticing that the girl is actually stalling for time, the Leader orders his right-hand man, a red-skinned monster nicknamed Madman, to get rid of her. Bruce, hearing this, loses his temper and turns into the Hulk. Having broken through groups of gamma soldiers, the monster reaches the Madman, who has locked Betty in a cell with the goal of turning her into a monster. Hulk destroys the installation and then defeats Madman. Betty, however, still turns out to be infected with radiation, which is why she faints, before asking the Hulk to take her to a military base.
As soon as the Hulk takes Betty to the base, the area is surrounded by a force field; The Hulk is incapacitated while trying to overcome the field. General Riker immediately orders the monster to be transferred to a laboratory, where he is injected with a drug that suppresses gamma radiation, and notifies General Ross that his daughter is fine. Betty later frees Bruce from captivity by knocking out a guard; Bruce, wearing his uniform, sneaks into the chemical laboratory and makes himself an antidote to the drug. The guards, however, notice Bruce and attack, forcing him to transform into the Hulk. The monster, smashing the base, reaches the force field generator, guarded by the gamma mutant Riker Flux, whom the Hulk throws into the generator during the battle, thereby destroying the force field, which allows him to escape. Back on Alcatraz, the Hulk, fighting his way through the Leader's gamma soldiers, makes his way under the island and destroys the installation that turns the Leader's soldiers into monsters. However, due to the explosion of a bomb placed by the soldiers, the Hulk turns into Bruce and is unable to transform for some time. Having walked through a huge part of the underground complex in human form, destroying another installation along the way, Banner comes to his senses and regains his ability to transform. The Hulk gets to the teleporter guarded by Vandal, where he meets the Leader himself - the same mutant, for whom, however, gamma radiation has increased mental indicators and the brain itself. After the Leader teleports to his lair, which he calls the New Freehold, the Hulk engages in a fight with Vandal, who does not want to return to human form, during which the cave partially collapses; both gamma mutants return to their human forms. Crawford, realizing what he has done, apologizes to Bruce. The latter follows the Leader into the teleporter.
Once in the New Freehold, Bruce finds himself trapped by the Half-Dead and the Madman. Having transformed into the Hulk, Banner fights back both opponents, after which, having made his way through the gamma soldiers, he gets to the Leader himself. The villain deliberately throws the sphere directly into the hands of the Hulk, after which it immediately absorbs all the gamma radiation, turning the monster into Bruce. Having told the scientist that he is now cured, the Leader orders him to leave, but Bruce, despite the Leader's psychic attacks, gets to the sphere and opens it, releasing all the gamma radiation.
