We are playing in the Sandbox mode with the Impossible difficulty setting, which is harder than hard. The Impossible difficulty setting have these game options:

Stamina: Enabled
Armor Penalty: Enabled
Rest Improves Moral: Enabled
Insane Damage: Enabled
Gore: Enabled
