I can trigger this bug with a chance of 40%.

All I do is hold the L button (xbox controller) to fast forward, it will trigger on enemy's turn. You can also trigger this bug by cornering the enemy, which I do most of the time. The bug can also be triggered on PS Vita as well. Very annoying bug.

Fix? Well release the L button on enemy's turn so the bug wont trigger. Or just avoid cornering the enemy.
