0If it doesn't work, destroy all your walls and doors and re-build them. Sometimes, there can be conflicts between buildings.
Also, make sure you ENTIRE PRISON is enclosed. If there is a hole somewhere ( or no closed door ), you will have this issue.
NOTE: Yard does not need to be FENCED. As long as it's outdoor and that your entire prison is enclosed, you can build a Yard.
The reason I have fences is simply for extra security. Also, you don't need a "door" perse.
The door in this issue is about a "PRISON DOOR" not being closed. It's not about a YARD door as Yard doesn't require doors.
The reason I had this issue is bcz of the conflict between the 2 fences ( bottom right ) + perimeter wall + Tower guard all position in a way that bugged out my yard.
Just destroy or relocate, then re-try. It should work.
