"The Dismal Oubliette" is probably the largest level in the original Quake. However, the first quarter or so of it had to be cut to fit the compiled level on a 1.44 MB floppy disk. The intro sequence was set in a dark underground cavern, continuing from the end of E2M5, "The Wizard's Manse". With the intro restored this map weighs in at 88 monsters on normal and a whopping 108 on hard.

This video shows the map as John Romero originally intended it, played on normal difficulty in the Qbism super8 port.
