realtime pip-boy glitch
I had to start Liberty Reprimed manually by entering in console commands that set the stage for the quest to start since for some reason "under the wire" didn't start and I couldn't give proctor ingram the tape to progress. I had my pip-boy open when I typed in the command that started the quest, and in order to start the quest the game has to teleport some NPCs and objects around so I had to go through a loading screen. When it was done I noticed my pip-boy was not only blank, but I also noticed some dude in the background moving, which isn't supposed to happen when you open your pip-boy.
you could probably replicate this by opening your pip-boy and typing "movetoqt [questID]", the game will teleport you to a new location and force a load, but this is just a guess; this might've just happened because i was in the same location the quest required me to be in so it "moved" me but it didn't move me, if that makes sense
