Alright, so here's what's going on:

My current setup of Artifacts includes the Artifact of Evolution, which causes enemies to gain permanent items every stage.
I also went out of my way to get 20 copies of the Lunar item, Eulogy Zero. Each copy adds a 5% chance that an item will be replaced by another Lunar item.
This combination allows enemies to be given Lunar items.

When the player obtains all four Heresy items, they are permanently transformed into the Heretic. This behavior also applies to enemies, bosses, and even the final boss!
After 32 stages, the pool for Evolution included all four Heresy items, which meant that every enemy became a copy of the Heretic!

This is normally impossible for Mitrhix, and it leads to some funny behavior, like entirely skipping the third phase of the fight (I was worried the game would softlock and not progress to the escape sequence :p)
Normally, even while playing as the Heretic, Mithrix will never steal Heresy items during the third phase, as it would disable all of your abilities, so this combination is the only way to see this interaction.

This transformation appears to affect every enemy in the game, though enemies with items (most notably Scavengers) will retain any passive or active items they spawned with, as well as their ability to use them.

However, there is one notable exception: the Void Reavers during the escape sequence seem to be hard-coded to spawn without any items, so they’re unaffected, even after all that.
