Resident Evil 3 is set in Raccoon City after most of its citizens have been turned into zombies due to an outbreak of the T-virus, a mutant virus secretly developed by the pharmaceutical company Umbrella. On September 28, 1998, former Special Tactics And Rescue Service (S.T.A.R.S.) member Jill Valentine meets up with fellow S.T.A.R.S. officer Brad Vickers shortly after being attacked by an intelligent bioweapon known as the Nemesis-T Type in her apartment. While trying to find a safe place, Brad is killed by a zombie and Jill is confronted by Nemesis once again. Umbrella Biohazard Countermeasure Service (U.B.C.S.) mercenary Carlos Oliveira saves Jill, explaining that he and his group of surviving U.B.C.S. mercenaries—Mikhail Victor, Tyrell Patrick, and Nicholai Ginovaef—have set up subway trains which they plan to use to evacuate civilians from the city. Jill helps them reactivate power to the subway and then departs on a train with Nicholai and Mikhail, while Carlos and Tyrell remain in the city to search for Dr. Nathaniel Bard, an Umbrella scientist who might know how to make a vaccine for the T-virus.

After Mikhail expresses his suspicions towards Nicholai on how their platoon was ambushed by zombies, Nemesis attacks the train and kills the civilians. Nicholai then locks Jill and Mikhail out, forcing them to confront Nemesis. When Nemesis grabs Mikhail, he sacrifices himself by detonating an explosive, causing the train to derail. Meanwhile, Carlos and Tyrell travel to the city's police department, where they plan to find Bard in the S.T.A.R.S. office. There, the two communicate via video with Bard, who informs them that he is at a hospital. Having survived the crash, Jill radios Carlos while being pursued by Nemesis. She manages to escape, but only after Nemesis infects her with the T-virus. Jill falls unconscious and Carlos finds her half a day later, taking her to the hospital where Bard is located. In the hospital, Carlos finds that Bard has been murdered. He also learns that Umbrella engineered the T-virus and that the company hired Bard to develop the vaccine. However, Umbrella's board now wants to destroy it and eliminate all traces of the virus' existence. After Carlos retrieves Bard's vaccine and administers it to Jill, Tyrell discovers that the U.S. government plans to destroy Raccoon City in a missile strike to eradicate the T-virus infestation.

Carlos travels to a lab underneath the hospital to find more vaccines, while Tyrell tries to contact whoever he can to try and stop the missile strike. Jill awakens on the day of the missile strike, October 1, and pursues Carlos to the lab. Tyrell then informs her that the U.S. government will not launch the missile strike if they can retrieve a vaccine from the lab, giving them only a few hours. Although Nemesis kills Tyrell shortly afterwards, Jill manages to synthesize a vaccine. She also learns that Nicholai is a supervisor hired by an unknown contractor to sabotage Umbrella's efforts to hide their involvement and to observe and collect data from attacks of several bioweapons, including Nemesis. An encounter with Nemesis prompts Nicholai to retrieve the vaccine from Jill as he leaves her to fight the monster. Jill eliminates Nemesis with a railgun and then meets up with Nicholai at a helipad. Nicholai destroys the vaccine, acknowledging that the city's fate is not important as long as he gets paid for sabotaging Umbrella. Carlos intervenes and restrains Nicholai, prompting Jill to shoot and injure him. Disgusted by his greed, Jill escapes the city with Carlos via helicopter, leaving Nicholai behind. As the city is destroyed by the missile strike, Jill vows to take down Umbrella at all costs.
