I was doing a stage in Trinity Universe a long while ago, when i tried to go out the exit, but right on the border of the exit where it usually requests you whether or not you want to leave, there was an item I had the max possible amount of.
When i walked into it, I accidentally said I didn't want to leave the dungeon yet, normally this makes you automatically take a few steps back, however I couldn't move because I was being told by the item i had exceeded the max capacity for it. So when that window disappeared, I walked right through the exit into deep space beyond the borders of the level. I paused the game and made a small video with my camera to show what it looked like. And then I walked through a wall and ended up back in the stage... I haven't been able to reproduce it since, because there hasn't been another item spawn right on the exit since then...
