Upon close inspection, the entirety of Doomsday Zone is completely unchanged in this hack. It seems that all of the Rings and meteorites you fly through are positioned exactly the same way.

The only big difference I managed to pull here compared to my S3&K run is getting a score-based 1-UP from landing one final hit on the Death Egg Robot before the ending starts.

The very end of the credits, as well as the title/cast screen you get for beating this game with all Super Emeralds, also made sure to change the game's logo a bit to reflect what this hack is.
