This is Sonic the Hedgehog on the NES courtesy of http://www.romhacking.net/hacks/3208/ - Originally a hack of the game Somari. This version replaces the Mario sprite with Sonic himself and blasts you into a close approximation of the Sega Mega Drive Classic.

To play you can grab a NES emulator and the IPS file from http://www.romhacking.net/hacks/3208/. After that grab LunarIPS from http://fusoya.eludevisibility.org/lips/ and Somari from http://www.freeroms.com/rom/nes/somari.htm. Open LunarIPS, click on "ApplyIPS", navigate to the IPS file followed by the original Somari ROM and you are ready to play!
