The only change in this hack is that there is an additional, invisible timer (which only runs when In-Game time runs) which starts at different but in general very large values, and whenever this timer reaches 0, Samus dies. And every time Samus dies due to the timer reaching 0, a new unknown value for the timer is chosen randomly.
