Basically at the moment it bugs out if you in any fashion reload the game during or after entering the code and pulling the Self Destruct Lever. Hopefully fixed soon! It will reload the game for you to enter the code, but will not reset the actual Lever, making it impossible for the event to trigger. Do Also Note, the game itself wont let you go back and pick a previous save, only showing you your quick save and your auto save.
