Silly Doomguy suicide by shooting barrels over a ledge. This has happened to me in a real longplay. Someone had stuck barrels over a ledge, shooting them would occasionally result in this once in a while hack where the barrels would go opposite of the shooting direction, so the explosion falling all over you. This is a real thing that makes the game fun when you shoot enemies on high ledges and once in a while one of them drops forward. I found out this was a chance based hack in the original game, it was intentionally put there to simulate enemies falling forward from ledges, just like in western films where the guy gets the bullet and falls forward for a cinematic. They did put that shit in the code, that once in a while, a vector opposite to the shooting direction would rather push the item shot, resulting in ridiculous ways to get injured or die like this (even in real gameplay, this is a demonstration map).
