There are 16 focus upgrades you can have but one of them (Intuition I) happens automatically so you only need to get 15.

You'll be rewarded 1 for lighting all 36 talking candles in the city.

The remaining six are not purchased or rewarded. Instead, they are picked up like collectibles. 3 of these are story-related and cannot be missed (or at least I'm _pretty sure_ they can't be missed). The other 3 need to be found and picked up manually.
The City: South Quarter District [In the fish-hawker's market area, enter the secret tavern, the focus symbol is behind the bar]
Chapter 3 [Find the secret trigger book in the Scriptorium area to access a hidden side room with a focus symbol]
Chapter 4 [Obtained near the end of the chapter as part of the story]
Chapter 5 [Check the dark nook at the bottom of old prison's stairwell to find the focus symbol just before you enter the reformation area]
Chapter 6 [Obtained near the end of the chapter as part of the story]
Chapter 7 [Obtained near the end of the chapter as part of the story]
Note to self: Candles #4 (4:07) and #8 (6:50) in the guide video for the candles are the same candle. So there arent 37 candles like this guy's videos show. Instead, there are only 36.
