I had already cleared this camp of ice-breath Lizalfos when the fire-breath Lizalfos briefly appeared out of nowhere.

Based on the above, I believe that the bug is most likely related to the in-game system whereby Lizalfos change from ice-breath to fire-breath depending on the time of day in Gerudo, but anything beyond that would just be speculation on my part.

According to redditors, the alternative element Lizalfos are actually underground waiting to spawn in, and the falling crate may have clipped their hitbox.
