Map 164 is a test copy of Britannia and as you can see, there the skull minidungeon south of Yew is fully accessible.
So, this is how it should have been, just when they decided to cut it, they simply removed that house that served as entrance. The question is, why? It seemed complete enough, it just required a treasure in the end and maybe some enemies (A skeleton in the column room and two blood bats in the final chamber?).
I always thought that the Gem of Death is what was meant there, but it's at Yew on this test map, too. So maybe some plot related would have been here and the simplification of the plot led to discontinuing this minidungeon.
