Someone or something definitely groaned... but who or what? I personally think it was probably Shane even though he seems just as confused as the rest of us. What do you think it was? It wasn't Molly - as I'm pretty sure she wasn't free from the electro shock room just yet. But the Devour asylum map is just creepy enough that there could be some weird groaning sound that happens when you're in this spot... so who knows? I only figured out the jumpscare sound in the basement at the beginning is supposed to signify that the wheelchair guy has vanished (not the best "clue" Devour developers... maybe make him vanish in front of the players and make the sound... that would work way better). It's possible that sound was Shane's cultist character leaning over and picking up the health pack. It's entirely possible that guy is in bad enough shape that he groans when picking up a medkit. I mean... he does look like he's seen better days.

What do you think? Was it Shane? Was it the map? Was it something else? Also, no making fun of my "groaning" at the end. That's just rude.
