If you put a shield gen and it is destroyed.
Next time you have ultimate, the primary ultimate will be Shield Gen instead of Teleport.
Since teleport is always the first to execute, if you press Q + M1 too fast, the game will not know which ultimate to effectively place and this glitch occurs.
