The only things that change on higher difficulties are the amount of lives you start with (Easy: 4, Normal and Hard: 2) and the stages you get. Easy is the original stages, Normal replaces one of the original stages with one of the new ones, and Hard replaces two original stages with both new ones. Enemy reaction times, amount of bullets on screen, and how much damage enemies take remains the same. I may get around to No Continuing them eventually, but realizing difficulty actually remained the same made me far less motivated to do so.
