To earn this you will need to wait until you start fighting the Exalt organisation when you do you will notice they are not as heavily armed as the alien forces but they are greater in number during operations.

They seem to have three types of soldier, regulars, rocket troops and medics. For this achievement you simply need to kill a medic with an explosive weapon. I pulled this off with a MECH using its grenade launcher. But I believe any explosive can do it as long as the medic is killed.

I managed to get three enemies together one of each type and then hit them with the grenade killing the medic and popping the achievement. Overall this isn't too hard at all just requires a bit of luck and a few explosives.
