The origins of Black Annis' story are shrouded in the mystery inherent to folklore. However, there are two women from the fifteenth century who have been suggested as potential inspirations for the legend. While it is unclear whether the tale of Annis existed during that time or if these women's lives became intertwined with the folklore, their stories offer intriguing possibilities.

The first candidate is Agnes Scott, a Dominican nun who lived as a forest anchorite—a hermit secluded in the woods. She wore the long black habit of her order and passed away in 1455. It is believed that Agnes resided in a cave near the Dane Hills and may have overseen a leper colony from there. A brass plaque in her memory can be found at Swithland's church, along with a three-foot veiled statue. The connection between Agnes Scott and Black Annis was proposed by Robert Graves, a poet and writer, although it remains uncertain whether this association was grounded in historical truth or poetic license. Nevertheless, it is an intriguing speculation.
