This is Doom 64 Retribution, Outcast Episode Level 3: Control, done on Watch Me Die-skill.

Control is the third level of Outcast Episode. If I got it right, this is a new map. Control has some pretty neat scripting stuff, a scanner and a combination switch puzzle, which means that you'll have to press four switches in correct order to get the Yellow Key.

It is not too difficult map to handle, although that puzzle might seem very confusing at first glance, but once you understand how it works, you should be able to solve it on your own.
