This is Doom 64 Retribution Absolution Episode Level 4, Shadows Watching. The original map was designed by Kaiser, and I believe Shadows Watching was the twenty-eighth level of Absolution TC.

I don't know if it is just me, but it seems this map has pieces of PSX DOOM MAP29: Twilight Descends found across the map...and I don't believe it is a coincidence...

I did find that inescapable lava pit, I don't know if it was meant to be lethal or not, but there is no way to get back up without cheating
