This is Doom 64 Retribution: Outcast Episode MAP07: Traps - done on Watch Me Die-skill with all kills and secrets.

Traps is the seventh level of Outcast Episode. There aren't that many traps here, but the one at the Exit can be quite deadly if not handled properly.

I spent some extra-time here looking for that Demon-artifact. After a brief period of disorientated and flustrated wandering, I found it. As I was picking it up, I got a massive lag, and thought the game crashed, but luckily it didn't xD.
