This is Duke Nukem Total Meltdown TC E4L7: Faces of Death - done with all kills and secrets on CGS.
Faces of Death is the lone secret level of Plug and Pray. Faces of Death is not originally part of this episode, in fact, it is known as E1L7 on PC, and it should be considered a Dukematch map.
Faces of Death has only 7 enemies - all of which are Mini-Battlelords, which coincidentally suits Plug and Pray's trend, but as a whole, these kind of maps don't really offer an awful lot apart from the high level of difficulty. It is also a disappointment that the devs decided to take the "easy way out" and include a slightly modified 3DR map instead of making a new level from scratch.
