This is Duke Nukem Total Meltdown TC E4L1: Nightmare Zone - done with all kills and secrets on CGS.

Nightmare Zone is the first level of Plug and Pray - Duke Nukem Total Meltdown exclusive episode. Nightmare Zone is indeed living up to its name. This Tomb Raider influenced map has very unusual puzzles and a high level of difficulty. There was a brief moment of confusion when I got "stuck" inside that water pool around the Yellow Door, and noclipped my way out, I forgot to shoot a switch that would lower the water level, so in hindsight using noclip was not necessary. We also get a new Pig Cop variant here, these ladies carry dual Pistols and wear a dress, hence the name; Pig in a Dress, as they are called. They appear only on this map.

I was struggling near the end but fortunately I made it through. Getting that last secret was very dangerous.

And that Tomb Raider had Bear suits, not "Beer" suits xD.
