John Henry Brennick and his wife Karen endeavor to cross the Canada–United States border to Vancouver in 2017 in order to have their second child. Karen wears a magnetic vest to conceal her pregnancy from security scanners because a stringent one-child policy prohibits a second pregnancy, even after the loss of the firstborn. A security officer observes the vest and sounds the alarm.
Brennick is apprehended, believing Karen to have escaped, and sentenced to thirty-one years at the Men-Tel Corporation's Fortress, a private 30-level maximum security penitentiary. As part of the prevalent prison-industrial complex, Brennick and other arriving prisoners are shown a video informing them that they will be used as slave labor to expand the Fortress. As a form of physical control and mental conditioning, all inmates are implanted with "Intestinators" that induce severe agony or death in order to maintain discipline. Director Poe runs the prison and supervises Zed-10, a computer that monitors daily activities. The prison is located underground in the midst of the desert, within a deep pit that can only be traversed by a retractable bridge. The prisoners are held in overcrowded cells protected by laser walls.
John is incarcerated alongside Abraham, a model prisoner who works as Poe's manservant and is awaiting parole; D-Day, an expert in machines and demolitions; juvenile offender Nino Gomez; and Stiggs, who attempts to extort John. John discovers that Karen has been apprehended and is being held on an upper level with their unborn child, who, as an illegal alien, is now officially Men-Tel's property and will be seized at birth.
Stiggs has a friend named Maddox who repeatedly intimidates John; the two are embroiled in a fight that ends with Maddox being shot and killed by a security turret. John captures Maddox's Intestinator and gives it to D-Day before being taken away and subjected to a mind-wipe procedure.
Poe, infatuated with Karen, assures her that he will release John from the mind-wipe chamber if she moves in with him. She agrees to assist John. Poe is revealed to be a cyborg with potent Men-Tel cybernetic enhancements. Four months later, a significantly pregnant Karen is able to use her access to the prison computer in Poe's quarters to restore John's mental faculties. Karen obtains a holographic map and gives it to Abraham in order for Abraham to give it to John. D-Day disassembles Maddox's Intestinator and uses a magnetic component to extract the Intestinators of the others.
During their next shift, John's group places their Intestinators in an air duct and stages a fight, prompting Zed to activate the devices and blast the duct open in preparation for their escape. Poe flushes the duct with steam and dispatches "Strike Clones," networked cyborgs equipped with flamethrowers and machine guns. Stiggs surrenders and is shot dead, while the remainder of the group kills a Strike Clone, steals its weapons, and uses them to destroy the remaining Clone Troopers.
Zed informs Poe of Karen's behavior. He informs her that her child, along with all other Men-Tel-owned infants, will be extracted through a fatal Caesarean section and made into a cybernetic. Abraham is strangled to death despite his and Karen's futile attempts to resist the cybernetic Poe.
John's party travels to Zed's control room by stealing a gun turret and converting it into an elevator. John kidnaps Poe and commands him to liberate Karen. Poe gives the order, but Zed refuses it, stating that Men-Tel does not negotiate during hostage situations. A gun emplacement then destroys Poe, leaving John's team without leverage. Once brought to the core computer, D-Day hacks into Zed and gains access to a potent feedback virus seized at the beginning of his sentence. After being shot and rendered incapacitated, he manages to activate the virus, initiating a complete system failure and causing all automated security to fail, resulting in the immediate escape of all prisoners. John and Gomez rescue Karen, hijack a truck, and flee to Mexico, where Karen gives birth to her and John's child in an abandoned barn.
