This chapter begins with Misaki and Madoka, but in Misaki's point of view. She tells Madoka that she thinks they will be the next to die. However, a girl dressed in black appears before Misaki and asks her, "Have you forgotten?"

Misaki is confused and doesn't remember. She picks up the camera at her feet. As she looks around, she realizes Madoka is no longer with her. She sets off to find her, and heads to the second floor office. Through the intercom, she hears Madoka crying and begging for help. Misaki examines the intercom and sees the call came from Room 203, with Madoka's name written next to it.

As Misaki leaves the room to go and find Madoka, a ghost appears before her and locks the doorway to the ward on which Madoka's room is located. She turns and makes her way down the steps, and sees two little girls in white dresses run by her, and head to the room under the stairs. She follows, and finds a picture of a grandfather clock underneath the stairs. She travels to the clock in the entrance hall and takes a picture of the base of the clock, and the picture reveals a four-digit number to unlock the door to the ward. Misaki returns to the second floor office, enters the digits, and the door unlocks. The ghost of the same nurse whom Ruka encountered earlier appears, and Misaki fights her off. She continues down to the patient wing to Room 203 to find Madoka.

She stops at Room 203, and hears crying from within. As she looks behind her, a door to another patient room opens and two ghosts appear. After she fights them off she realizes Madoka's room is sealed with spirit energy. As she takes a picture of the door, the picture shows an image of a room with mirror- the same mirror in the room full of books, the anteroom off the museum. Misaki leaves the ward and proceeds down the stairs, and a ghost of a little girl in a dress with long black hair appears at the top of the stairs, saying "Die."

Misaki returns to the anteroom off of the museum, and examines the mirror. A ghost of a man appears behind her, and she fights him off as well. She finds the key to Madoka's room in front of the mirror. As she exits the room, a female ghost in all black, with flowing clothes and hair and a blurred face, appears in the corner. Misaki finds a piece of a mask in the display case, near where the woman was standing. Misaki leaves and heads towards Madoka's room again. Right before she begins climbing the staircase, the ghost of a younger Madoka lies at the foot of the staircase, curled up. At the top, the same little girl with long hair and a dress stands at the top.

Misaki continues to the patient wing, but stops by the second floor office again. A nurse appears before her, and the intercom starts up again, but the only thing heard is noises from Room 207, the room with "Ayako" written beside it. Misaki leaves, and uses the key to enter Madoka's room.

Madoka's room has drawings all over the walls, and a diary on her desk she wrote when she was here as a little girl. One of her entries describes how she feels ill, her eyes burn, her head aches. In her diary, Madoka mentions that "She wants people like Ayako to leave this world," but refuses to take matters into her own hands because she feels she will lose a part of herself. She prays for sleep.

Misaki heads into Madoka's bedroom, and sees Madoka sitting calmly on her bed. As she reaches out to touch her, Madoka vanishes and leaves behind a note in confused writing about how she can no longer see and writes down screaming sounds. As Misaki turns to leave, Ayako peers around the corner and laughs at her.

Misaki leaves the room, and a nurse appears before her, walking down the corridor. Misaki follows her as she disappears, but reappears again being dragged on the floor by Ayako, who has a firm grip on the nurse's hair as she pulls her towards her room. Misaki follows, and enters Ayako's room.

Mannequin pieces cover Ayako's room. Torn-off arms and legs of dolls hang from the ceiling, bodies of mannequins stand on her desk, and are toppled over on the floor. As Misaki proceeds through the room, a light fixture begins to turn and Misaki heads towards it. In a moment of silence, Ayako pounces on Misaki and begins to attack.

After Ayako is beaten, Misaki has another memory of the girl she saw in the beginning of the chapter, with short black hair dressed in all black. They speak about a promise to always be together
