The basics and some oddities with the mechanic. It's unnecessarily confusing in my opinion. To start off, triggering a Breaksight deals an arbitrary amount of damage based on yet undisclosed calculation, and if the target's current HP is lower than that amount, Killsight pops up in its place. 2 of the same thing really.

Breaksight and Killsight is gonna be referred to just as "sight" here for the sake of simplicity.

=========================
The normal/private sight. Term used is not official.

The normal sight appears based on enemy's action or when stunned. I'm demonstrating with Gilgamesh for it's sight pattern. It can be interrupted from appearing if hit too early, such as in a combo. The target must also be locked-on. The reason I tagged in the "private" term is that only the player can trigger it. Not in vid, if an ally hit a normal sight, the sight disappears.

=========================
The visible/public sight. Term used is also not official.

This sight appears on certain situations: ambush, enemy after Rage, inflicted by the magic Death, the Rursan enemies, etc.. I mentioned "public" because the AI can actually trigger them, and the target does not need to be locked-on.

=========================
Oddities #1

Depending on how the game calculates (or miscalculates) it, Breaksight can kill, which is quite normal. In the clip with the Iron Golem, it survives a Killsight probably due to conflict with it gaining Invincible status before it dies, even though it never got a chance to buff itself (animation-wise).

=========================
Oddities #2

In the turret section with Shinryu, I managed to trigger a sight for the same damage normally dealt. To my knowledge, nothing pops up sight in that part. In the equally dull Bahamut Zero vs Shinryu section, triggering the sight "correctly" launches a scripted take down. But if I were to keep holding the attack button...

=========================
In an unrelated side note, I did notice there are ways to increase the damage done through sight. Did a number of tests, but the findings make no sense. So I don't bother to include them anywhere, anytime. I've clocked in over 100 hours, and am honestly quite sick of it.
