I hope this video helps figure out how to turn on buttons and add them. sorry I couldn't add commentary. Any questions just leave a comment.

Tap settings then tap modify, press add buttons button on top, then go down the list and turn on the buttons, then go to options and turn on Crouch/ jump.
Go to customize controls and find kick at the bottom of list press enter and then press the button you want to use as kick.
now try pressing Crouch and then kick at the same time to slide.
