These issues happen with Slim PAL PS2 (PS2 emulates PS1 or it doesn't, who knows if my internets are correct) with my PAL version and with emulators apparently with PAL and NTSC/J versions (probably with NTSC/U too, haven't occured to me so far). Never seen them occuring on actual PS1.

Problems that occur sometimes:
Boxes missing - seen here with box 9 on Orient Express
Boxes misplaced - seen here with box 33 on Midnight Run

I'm not a big fan of emulation issues.
