There is code in the current GIT version of MAME that can be enabled at compile time allowing for MAME to do full Low Level Emulation of the QSound DSP, running it as an actual CPU. For some further details see https://rants.vastheman.com/2018/03/20/qfactor/

The QSound DSP is actually magnitudes more powerful than the rest of the system, so on a 4Ghz i7-4790K it takes the emulation speed from around 2000% unthrottled to 130%, so the Low Level Emulation is VERY demanding at the moment. It might be possible to win some speed back with a recompiler, but proper emulation of the chip is always going to have much higher requirements than the inaccurate HLE.

This video shows Street Fighter Zero 2 Alpha running with the Low Level Emulation. The effect is subtle, and this is not meant to be a demonstration of good play (I'm terrible at these games)
