When you send Greirat to pillage an area, the condition you have to meet in order to make him return to Firelink is just to kill a boss. However, Greirat will die in Irithyll if you don't do it right.

In order to make sure he survives, you either have to have rescued Siegward from the well at the Cathedral of the Deep, or have gotten Patches to Firelink and NOT bought Siegward's armor from him. You can only do one or the other, as you need Siegward's armor to rescue him.

If Patches has the armor, you have to have told Patches about Greirat going to Irithyll for him to save him. If Siegward was rescued, he must be IN Irithyll at the time that Greirat is and when you beat the next boss. You can find Siegward just past the sewers on the way to Pontiff's boss room. If you advance to Irithyll dungeon, Siegward will have moved from his spot in Irithyll and will be locked in a cell in the dungeon, so he won't be able to save Greirat.

I rescued Siegward, sent Greirat to pillage, met Siegward in Irithyll, and killed Pontiff before making Siegward move. You can also touch the first Irithyll bonfire, send Greirat to pillage, and go back to kill the Old Demon King in order to make Greirat return early.
