I was playing vanilla Half-Life from Steam; not Source, and without any mods or file changes. I had just downloaded the game and verified my files.

Destroying this pipe from below with the crowbar makes you stuck. The only way to get out of this scenario is to load your game. This is probably due to the debris of the destroyed pipe instant-spawning on top of the floor without any movement. If the debris "fell" on the floor, then probably Gordon would get crushed.
