I was playing vanilla Half-Life from Steam; not Source, and without any mods or file changes. I had just downloaded the game and verified my files.

In the Questionable Ethics chapter, after you enter the Lobby of this laboratory, the game applies a weird reverb effect to all audio that makes the right sound channel loud, and the left sound channel quiet and muddy. This is the most disturbing and distracting of all the bugs I've found. Restarting the game seems to fix the problem, as long as you don't return to this lobby.
