In Baator ypu meet the Pillar of Skulls and have to make a choice, you can sacrifice yourself or a party member or give up the location of Fhjull. I didn't want to lose a party member or betray Fjhull after he'd helped us so I sacrificed myself, permanently losing 15 HP in the process, ouch!.

FYI settings for this run were difficulty 4, no full heal on rest and no max HP on level up.
