John rescues Esther while orderlies drag Kate off to another ward and drag her away. As John takes Max and Esther home, doctors sedate Kate.

As John pours himself a very stiff drink and tries to relax in the den, Esther—now in makeup and a clingy black negligée—brings John a cheese and wine snack and begins to seduce him. John is horrified to realize Kate was right in her insistence that there is something very wrong with Esther; he pushes Esther away and threatens to send her back to the orphanage for this kind of behavior. Esther breaks down crying and runs back to her room in despair. Meanwhile, as Kate is coming out of sedation, she receives a call from a doctor at the Saarne Institute. He tells Kate that she is in great danger, and that 9-year-old "Esther" is in reality a 33-year-old woman named Leena Klammer. She has hypopituitarism, a condition that stunted her physical growth, and has spent most of her life as a serial killer posing as a little girl who tricks people into adopting her, then turns on them when she doesn't get what she wants from them. The doctor tells Kate that Leena is extremely violent and has murdered at least seven people, and that she bears scars on her neck and wrists, which Esther always kept covered, that she received while trying to escape her straitjacket. Among her victims were an American family that adopted her in Estonia and brought her to the USA, but she killed everyone in the family and burned their house down because the father rejected her sexual advances. Kate suddenly realizes that John is in great danger and, though still woozy from the sedatives, she bids farewell to the doctor and begins disconnecting herself from medical pumps and monitors in an attempt to get back home before it is too late.

Back at the house, Esther/Leena's despair has turned to rage after being spurned by John; she ransacks her room, then wipes away her makeup and begins to remove body wraps, false teeth, and ribbons from her neck and wrists that hid her scars and enabled her to better pass as youthful Esther. Now fully transformed into Leena, she breaks into Max's room and hides the girl's hearing aids in case Max wakes up and tries to find out what is going on, then heads out to hunt for John—not noticing that Max has awakened and watches her leave. John finds his way up to Esther's room and discovers the macabre images hidden in the fluorescent paint, including a stylized portrait of a pair of lovers painted on the wall behind all of Esther's childlike artwork. Horrified further, John rushes out of the room at the sound of the house phone ringing. He recognizes Kate's number on the cordless handset display and is just about to answer it when Leena stabs him with a knife and leaves him for dead. Max witnesses this, and hides.
Kate, unable to get John on the phone, calls the police en route to her own home, then arrives back at the house and rushes inside to find her family. She finds John stabbed on the floor, either unconscious or dead. Leena gets a gun from John's safe and shoots Kate in the arm, then goes searching for Max, finding her in the greenhouse. While Leena shoots at Max, Kate crawls onto the greenhouse roof, breaks through the glass above Leena, and lands on her, knocking her unconscious. Kate takes the gun and leaves the greenhouse with Max, not realizing that Leena has managed to escape from the remains of the roof on the floor and is now stalking them again.
Kate and Leena both run into the snow covered forest as Kate desperately tries to escape from her with Max. Leena runs after her and they end up fighting on the frozen lake with Leena trying repeatedly to stab her. Max then takes the gun Leena dropped and shoots the frozen lake, which makes a hole that Leena and Kate fall in. Leena, hiding a knife behind her back, pretends to revert to her little-girl persona, begging, "Please, Mummy, don't let me die." Kate angrily responds, "I'm not your fucking mummy!" then kicks Leena in the face, snapping her neck and sending her back into the pond to drown. Kate and Max are then rescued by police.
