Since I've seen a lot of people confused about the Katana, I hope this description will alleviate some confusion.

Saves from new game to all of their subsequent Next Fears will always stack from their previous games' playthroughs.

If that's confusing terminology, then think about it like this: If someone were to get the good ending on a brand new game and get another good ending on Next Fear, then they'll unlock the Katana after they get a bad ending on the next Next Fear.

On Next Fear, you just want to get a bad ending if the ending you got previously was a good ending, and vice versa. They need to be on subsequent saves. Think of the saves as stacking on top of each other. It doesn't matter if they're on different files on the file select, so you can save your Next Fear game below your previous game (if you wish to keep your previous saves), and it'll still count.

Once you unlock the Katana, it'll then be a permanent Next Fear unlockable on any Next Fear playthrough. So, for example, you can start a brand new game (so not Next Fear) and it'll automatically unlock on the very next game (so your new Next Fear game) without you needing to get both endings.

These items can only be acquired on Next Fear mode. Also, New Fear is actually called Next Fear. I just like calling it New Fear as a pun on New Game+ and Next Fear, as Next Fear is just a NG+. Sorry for being confusing.

I didn't bother mentioning the Yellow Hyper Blaster because it's the same item as the Red Hyper Blaster, just more powerful. That'd be like if I counted each and every individual level from 1-to-Max to be a different weapon in a Ratchet and Clank "All Weapons" video.
